This policy explains how we handle personal information collected through cityortho.ca, our online forms, our Facebook and Instagram ads, and when you contact us. Personal health information we collect while providing orthodontic care is also protected under Ontario's Personal Health Information Protection Act, 2004 (PHIPA).
When you fill out a form on our website or a lead form on Facebook or Instagram, we collect the details you provide: usually your name, email address and phone number and, if you choose to share them, your preferred appointment date, who the treatment is for, your preferred way to be contacted and any message you send. Referring dentists and other professionals may also send patient details and records through our referral form.
When you browse our website, we and our service providers automatically collect limited technical information, such as the pages you visit, your approximate location, your device and browser type, and how you arrived at our site.
We use your information to respond to your inquiry; to book, confirm and remind you of appointments; to contact you by phone, email, text message or WhatsApp as you have asked; to provide and coordinate your care; to understand how our website and advertising perform so we can improve them; and to meet our legal and professional obligations. We do not sell your personal information.
By submitting a form, you agree that we may contact you about your request. If you opt in to offers and updates, we will send occasional orthodontic tips and offers, and you can unsubscribe at any time. You may withdraw your consent at any time by contacting us, subject to legal or contractual restrictions.
Our website uses cookies and similar technologies from Google (Google Analytics, Google Tag Manager and Google Ads) to measure visits and the results of our advertising.
